Rahu in the 6th, Ketu in the 12th
Mahapadma is Kaal Sarp with Rahu in the 6th house (illness, debt and rivals) and Ketu in the 12th (expenses, foreign lands and moksha). It forms only when all seven grahas sit on one side of that line. Kaal Sarp and Kal Sarpa are two spellings of the same dosha.

With Rahu in the 6th, tradition reads a pull between illness, debt and rivals on Rahu’s side and expenses, foreign lands and moksha on Ketu’s: one end wants more, the other lets go. It is felt most in the Rahu and Ketu dashas, and it is read with the whole kundli, never alone.
Kaal Sarp is not among the yogas of Brihat Parashara Hora Shastra; the twelve names come from later tradition, which is why astrologers weigh it differently.

PhaladeepikaPhaladeepika 8.33 reads Ketu in the 12th house (the house of loss and expense) as a caution about conduct and money. The text warns of wrongdoing in secret, spending on base things, wasting wealth and improper conduct, and cautions about ailments of the eyes.
Lal KitabKetu in the 12th house opens the way to moksha and spiritual liberation. It brings foreign travel for spiritual purposes and is good for meditation.
